VeriSureDX at-home collection kit
Collect your sample at home and send it to the lab in four phases. Read every step before you open your kit.
Before you begin
Collect Monday through Thursday, first thing in the morning for a fasting blood draw. Be sure to drop off or have UPS collect your blood sample before noon so your sample ships the same day for our lab to receive it the next day.
Drink extra water the day before. Fast for 12 hours before you collect your sample. Be sure to collect your sample first thing in the morning for a fasting blood draw.
Please send this card back to us in the Biohazard bag, along with your collected sample.
Drop the package at a staffed UPS location. Never use a drop box. Or schedule the day before with UPS for pick up. Download the UPS app to do so.
Your kit
Lay everything out on a clean, flat surface and check it against this list.
You may also need a razor, a mirror, and a 5-minute timer.
Steps 1 to 15
Take your time. If something looks wrong at any point, stop and contact us before you continue.
Drink two 8 oz glasses of water about 30 minutes before you collect.
Fill out the card completely, including the collection date and time of when you collected your sample.
If needed, shave the top of your arm - the Collection Area - so the device properly stickers to and sits flat against your upper arm.
Wash your hands, then lay out your kit on a clean, flat surface.
Open the Collection Device and Remove Tube Cap. Place the tube cap somewhere safe to recap the tube. Connect the tube to the spout of the device. Make sure the tube line markers are facing outward so that you can see when the tube is filled.
Activate the heat pack by bending the metal activation tab. Once heating is activated, hold it on the Collection Area for about 4 minutes. If the upper arm stays cold, rub the area for 2 minutes instead.
Wipe the area with an alcohol pad and let it air-dry.
Remove the sticker cover on the flat side of the device. With your other hand (the arm you didn’t clean), press the sticker part of the device onto your cleaned upper arm Collection Area, with the tube pointing down. Let your arm hang at your side.
Follow the steps for the device in your kit.
Remove the clear plastic cover from the red round button and press all the way down once. Set a 5-minute timer. Blood may take 1 to 3 minutes to appear in the tube.
Remove the safety clip and push the switch to the bottom position. Set a 5-minute timer. Blood may take 1 to 3 minutes to appear in the tube.
When the tube is filled to the top line, peel the device off your arm. Use the gauze pad and bandage if needed, then twist the tube free and cap the tube.
Ensure the cap is tightly on the tube. It may be either a snap cap or a twisting lock cap. Gently tip the tube upside down ten times to mix the preservative. Do not shake.
Place the tube and your completed DirectDX Collection ID Card in the biohazard bag, with nothing else inside.
Place the packed biohazard bag into the preaddressed Return Shipping Bag that came with the box and seal to close.
Drop your Return Shipping Bag to a staffed UPS location. Never use a drop box. Or prepare it for UPS pick up if pick up was scheduled ahead. Drop off or pick up must happen before noon to ensure proper overnight shipping. The Return Shipping Bag must ship the same day, overnight delivery, as when you collected your sample. UPS Pick ups can be scheduled with the UPS app.
Dispose of used materials safely, reuse nothing, and wash your hands.
